We are excited to be one of 190 Physician Group Practices chosen to participate in the Centers for Medicare & Medicaid CMS Oncology Care Model (OCM) 5-year pilot program.  Participation in this pilot allows our practice to submit feedback that will help shape the future of Oncology care.  The Oncology Care Model Pilot requires participants to offer enhanced services, many of which our practice has offered for many years such as:

  • Provide 24/7 access to an appropriate clinician who has real-time access to patients’ medical records
  • Use Certified EHR (Electronic Health Records) technology
  • Use data for continuous quality improvement
  • Provide core functions of patient navigation
  • Use therapies consistent with nationally recognized clinical guidelines

In addition to the above services, we have implemented a Navigation Program utilizing a Certified Oncology Nurse Navigator.  Augusta Oncology strives to guide patients through the cancer journey as seamlessly as possible with our clinical, pharmacy and business office staff.   The Nurse Navigator can provide additional services such as:

  • Care Management Plan Review
  • Survivorship Care Plan Review
  • Central point of contact for complex situations
